# IL 301: Pilot Report 提交檢查表 / Submission Checklist

在提交 Pilot Report 前，請逐項確認：

- [ ] 已明確引用一個具體的核准版本（IL 201 Evidence Review 或 IL 202 Readiness Review 決策紀錄），不是團隊私下的最新草稿。
- [ ] RACI、停止權持有人與觸發條件，以及正式 Pilot 期間與檢查點都已明確填寫。
- [ ] 已重新驗證有效權限與資料範圍，並記錄具體驗證方式，而非直接沿用 IL 202 的舊結論。
- [ ] 架構與資料／治理控管已對照最小監測資訊集，且監測範圍沒有超出核准的資料範圍。
- [ ] 每一種錯誤類型（工具失效、資料缺漏、資料矛盾、低信心、越權要求）都有對應的人工接手觸發條件。
- [ ] Pilot Log 記錄完整，包含時間、版本、任務範圍、結果、例外與接手、影響與處理。
- [ ] 評估報告分開報告品質、時間、成本，並誠實揭露工作量、案件難度組成、人員配置等干擾因素。
- [ ] 至少檢查過一次評估報告的結論是否忽略了干擾因素（見課程模組 6 範例）。
- [ ] 已完成停用、復原、撤權三種治理演練並記錄實際耗時；例外處理紀錄含範圍、期限與核准人。
- [ ] 交接與支援套件備齊，並誠實列出至少兩項殘餘風險，含目前緩解措施與仍然殘留的部分。
- [ ] Adoption Gate 建議是 SCALE／LIMITED ADOPTION／ITERATE／HOLD／STOP 五者之一，並附至少三項具體理由。
- [ ] 建議中已寫明成本與後續責任歸屬（誰負責維運預算、誰複核殘餘風險、下次審查日期）。
- [ ] 已誠實列出本次 Pilot 尚未涵蓋的範圍或已知限制。
- [ ] 若為合成練習，已標明「合成練習，不等同正式專案」；若涉及真實企業資料，已依組織資料政策取得授權。
- [ ] Pilot Report 內容視為敏感資料，僅存放於受管理位置並只分享給經授權人員。

Before submitting a Pilot Report, confirm each of the following:

- [ ] A specific approved version (an IL 201 Evidence Review or IL 202 Readiness Review decision record) is explicitly cited, not the team's own latest private draft.
- [ ] The RACI, the stop-authority holder and its triggers, and the formal pilot period with checkpoints are all explicitly filled in.
- [ ] Effective permissions and data scope have been re-verified, with the verification method recorded, not simply reused from IL 202's old conclusions.
- [ ] The architecture and data/governance controls are checked against the minimum monitoring information set, and monitoring scope does not exceed the approved data scope.
- [ ] Every error type (tool failure, missing data, conflicting data, low confidence, out-of-scope request) has a matching human-handoff trigger.
- [ ] The pilot log is complete, covering time, version, task scope, result, exception and handoff, and impact and handling.
- [ ] The evaluation report reports quality, time, and cost separately, and honestly discloses confounders such as workload, case-difficulty mix, and staffing.
- [ ] You checked at least once whether the evaluation report's conclusion ignores a confounder (see the course Module 6 example).
- [ ] The three governance drills (disable, recovery, revocation) are complete with actual timing recorded; the exception-handling record includes scope, expiry, and approver.
- [ ] The handoff and support package is complete, and at least two residual risks are honestly listed, each with current mitigation and what remains.
- [ ] The Adoption Gate recommendation is one of SCALE / LIMITED ADOPTION / ITERATE / HOLD / STOP, backed by at least three concrete reasons.
- [ ] The recommendation states cost and ongoing-responsibility ownership (who owns the operating budget, who reviews residual risk, and the next review date).
- [ ] Scope not yet covered by this pilot, or known limitations, are honestly listed.
- [ ] If this is a synthetic exercise, it states "synthetic exercise, not equivalent to a formal project"; if real company data is involved, it has been authorized per the organization's data policy.
- [ ] The pilot report is treated as sensitive: stored only in managed locations and shared only with authorized people.
